Sign up to play in the Student VS Staff Softball Game on Thursday, September 22, 2022. Filling out the form below makes you eligible to play in the game and acts as your confirmation to the liability statement below:
I hereby release and hold harmless Hagerstown Community College, its Trustees, officers, directors, faculty, coaches and employees; participants; owners and lessees of premises used to conduct the event, their officers and employees from and against any and all liabilities to the undersigned, his/her dependents, assigns, personal representatives, heirs and next of kin for any and all damages, expenses (including attorney fees), claims, judgments, actions or causes of action as a result of any loss or injury to the person or property that I may sustain or suffer during, or arising out of, the course/activity.
I understand that it is my responsibility to have appropriate insurance to cover any loss or injury to person or property.
I know, understand, and appreciate the risks associated with participation and I am voluntarily participating in the activity. In doing so, I am assuming all of the inherent risks of the sport. I further understand that in the event of a medical emergency, an HCC representative will call EMS to render assistance and that I will be financially responsible for any expenses involved.
